DATA SUBJECT APPLICATION FORM
GENERAL INFORMATION
Law No. 6698 on the Protection of Personal Data (the “Law”) provides, in Article 11, that individuals whose personal data are processed and who are defined as data subjects under the Law (the “Data Subject”) may exercise the rights granted to them in relation to their personal data and must apply to the Data Controller to exercise those rights.
Pursuant to Article 13(1) of the Law, applications concerning these rights must be submitted to Falcon Tactical Range Club Silah Eğitim ve Güvenlik Hizmetleri Limited Şirketi (the “Company”), acting as the Data Controller, in writing, by email over the internet, or by other methods determined by the Personal Data Protection Board (the “Board”).
APPLICATION METHODS
You may submit your application to our Company through any of the following methods:
i. By personally submitting a fully completed copy of this Application Form bearing your handwritten signature, together with a document enabling verification of your identity, at Merkez Mahallesi Begonvil Sokak Nef 03 A ve B Blok 1/1I Kağıthane/İstanbul.
ii. By sending a fully completed copy of this Application Form bearing your handwritten signature, together with a document enabling verification of your identity, through a notary public to Merkez Mahallesi Begonvil Sokak Nef 03 A ve B Blok 1/1I Kağıthane/İstanbul.
iii. By signing this Application Form with a “secure electronic signature,” as defined under Electronic Signature Law No. 5070, and sending it to [email protected].
iv. By completing and signing this Application Form, scanning the form bearing your handwritten signature, uploading the scanned copy to a computer, and emailing it to [email protected]. If you choose this method, a document enabling verification of your identity must also be attached to the email.
The channels specified above constitute “written” application channels pursuant to Article 13(1) of the Law. Once other methods determined by the Board have been announced, the Company will announce how applications may be received through those methods.
Pursuant to Article 13(2) of the Law, applications submitted to us will be answered, depending on the nature of the request, “as soon as possible and no later than thirty days” from the date on which we receive your request. Requests are, as a rule, handled free of charge. However, if fulfilling the request entails a cost, the Company may charge a fee pursuant to Article 7 of the “Communiqué on the Procedures and Principles for Applications to the Data Controller,” which provides as follows:
> “If a written response is provided to the data subject’s application, no fee shall be charged for up to 10 pages. A processing fee of TRY 1 may be charged for each page exceeding 10 pages. If the response to the application is provided on a recording medium, such as a CD or a flash drive, the fee that may be requested by the data controller shall not exceed the cost of the recording medium.”
This form and any information and documents requested from you according to the nature of your request must be provided to us completely and accurately. If the requested information and documents are not duly provided, difficulties may arise in conducting a complete and thorough investigation by the Company in connection with your request. In such circumstances, the Company declares that it reserves its legal rights. Accordingly, this form must be submitted in full, including the requested information and documents appropriate to the nature of your request.

If any requested information or documents are missing, you will be required, upon our request, to complete and submit the missing information and documents to us. If the application cannot be processed because the information and documents have not been provided in full, a response explaining this situation will be provided within the specified thirty (30) days.
